• Be at least 55 years old
• Live in Senior CommUnity Care PACE service area (see below)
• Eligible for nursing home level of care as determined by the State of Colorado
• Able to live safely in the community with PACE services at the time of enrollment

...if the PACE Interdisciplinary Team determines that a Medicaid eligible participant needs to live in an assisted living or nursing home, the participant will pay for these housing costs with their own income (less personal needs dollars). PACE will pay the difference between income and cost of assisted living or nursing home. This does not apply to participants who are paying PACE with private funds.
