What is PACE?

PACE stands for Program of All-Inclusive Care for the Elderly. The model is centered on the belief that the well-being of seniors with chronic care needs are better served by providing health care and supportive services that enable them to remain living in their home whenever possible.

Senior CommUnity Care provides for all the health care and social needs of older adults who enroll in the program as determined by an interdisciplinary team of health care professionals. These professionals include physicians, nurses, physical and occupational therapists, dieticians, drivers, social workers and recreational specialists.

The PACE Day Center is a physical location where medical and support services are provided in addition to meals and individual and group social activities. The Day Centers are located in Montrose and Eckert with a satellite site in Paonia.
