Our Team

The Senior CommUnity Care team includes health care professionals who know how to manage the care of the elderly. They will create a personal health care plan. This plan will help monitor changes in health care needs. It also helps manage all health care services and provide support for clients in the home and our Center. The team includes:

Our Management Team

Craig Ammermann

Regional Director of Operations

Craig Ammermann joined the Senior CommUnity Care team as the regional director of operations after working with the Senior Meals Program on the Western Slope. Prior to relocating to the Western Slope, Ammermann worked in rural communities in both Minnesota and Iowa for a large, non-profit faith-based organization as a nursing home administrator. He graduated from St. Mary’s University of Minnesota with a Master of Arts degree in health and human services. He earned his bachelor’s from Bethel College in North Newton, Kan. Ammermann is also a Licensed Nursing Home Administrator.

Pam Cook R.N. Senior CommUnity Care-PACE Executive Director

Prior to joining Senior CommUnity Care in 2009  as the Montrose Day Health Center Clinic Director, Pam worked as a nurse at Montrose Memorial Hospital.  In October of 2010 she took on the role of Center Director and has recenlty been appointed the Executive Director. Pam brings a wealth of knowledge and experience to Senior CommUnity Care.

 

Dory Funk, M.D.

Dory Funk, M.D., is the medical director at the Senior CommUnity PACE Health Center in Eckert. He received his Doctor of Medicine from the University of Arizona. He is a member of the Alpha Omega Alpha National Medical Honor Society. Dr. Funk completed his residency at St. Mary’s Family Medicine in Grand Junction. He is board certified in family medicine. Currently, Dr. Funk runs a private practice in Paonia and Hotchkiss. He serves on the board of directors of Delta County Memorial Hospital. Dr. Funk was the medical director of North Fork Ambulance Association for 10 years prior to opening his practice.

Dorothy Pew Delta CountyDay Center Manager

Dorothy Pew is the Day Center Manager for the Senior CommUnity Care PACE Health Center in Eckert. Prior to joining the Senior CommUnity Care family Dorothy had over 10 years of experience in business management development specializing in geriatric healthcare. In her previous life she devoted many years to managing not for profit companies in rural communities focusing on housing, economic development and youth training programs. She and her husband have lived in Delta County for 8 years.

Debby Harrison-Zarkis Montrose County Day Center Manager

Debby Harrison-Zarkis is the Day Center Manager for the Senior CommUnity Care PACE Health Center in Montrose. Debby served as the case manager for Senior CommUnity Care for 2 1/2 years. Her experience includes 5 years as a case manager for Community Options serving developmentally disabled persons and 5 years as a social worker for Montrose County most recently in the Adult Protective Services Division. Debby and her husband live in Montrose and enjoy life with their two very active teenage sons.

Monica Rawson

Monica Rawson N.P. Nurse Practitioner

Monica Rawson joined the Senior CommUnity Care staff in February 2009 as the nurse practioner. She is a board certified family nurse practitioner with more than 14 years of experience. Rawson graduated from Mesa State College with a Bachelor of Science in nursing. She earned her Master of Science in nursing from the University of Colorado, Colorado Springs. Prior to coming to Senior CommUnity Care Rawson worked in acute care settings on the Western Slope. She is committed to caring for the geriatric population and has lived on the Western Slope for most of her life.

Carol Keller

Carol Keller, RN

Quality Assurance Manager

Carol Keller oversees the quality assurance program for Senior CommUnity Care, assuming responsibility for compliance with state and federal regulations. Keller has experience in all aspects of nursing from long-term care as a director of nursing, to hospital, hospice and home health care environments. Along with 15 years of nursing experience, she has experience in quality project management and technical writing. Keller holds an associate’s degree in nursing and a bachelor’s degree in molecular biology. She is also certified in Life Care Planning, projecting life time services for persons with catastrophic injury. Keller has lived in Delta County for 15 years, providing quality health care to those residents.

Jacque Adragna

Marketing/Enrollment Supervisor

Jacque Adragna is the Marketing and Enrollment Supervisor for Senior CommUnity Care. Jacque has been with the program for over 3 years, since the program’s beginning in 2008 in the Enrollment Department. Jacque earned her Bachelor’s in Kinesiology with an emphasis in Exercise Science from Mesa State College and is currently working on her Masters in Health Care Administration.
