What is PACE?
PACE has been a nationwide program since the 1970s. It began as a grassroots program but was supported and funded by Medicare. It has proven to meet the needs of elders seeking a complete health care system that helped them to stay home. There are about 50 PACE programs nationwide today.
Senior CommUnity Care is the Western Slope’s first PACE provider. We manage all the care a senior needs so that they can stay home. This helps them avoid a higher level of care, such as a nursing home. We think it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.
