Our Team
The Senior CommUnity Care team includes health care professionals who know how to manage the care of the elderly. They will create a personal health care plan. This plan will help monitor changes in health care needs. It also helps manage all health care services and provide support for clients in the home and our Center. The team includes:

Our Management Team
Craig Ammermann
Regional Director of Operations
Craig Ammermann joined the Senior CommUnity Care team as the regional director of operations after working with the Senior Meals Program on the Western Slope. Prior to relocating to the Western Slope, Ammermann worked in rural communities in both Minnesota and Iowa for a large, non-profit faith-based organization as a nursing home administrator. He graduated from St. Mary’s University of Minnesota with a Master of Arts degree in health and human services. He earned his bachelor’s from Bethel College in North Newton, Kan. Ammermann is also a Licensed Nursing Home Administrator.
Dory Funk, M.D.
Dory Funk, M.D., is the medical director at the Senior CommUnity PACE Health Center in Eckert. He received his Doctor of Medicine from the University of Arizona. He is a member of the Alpha Omega Alpha National Medical Honor Society. Dr. Funk completed his residency at St. Mary’s Family Medicine in Grand Junction. He is board certified in family medicine. Currently, Dr. Funk runs a private practice in Paonia and Hotchkiss. He serves on the board of directors of Delta County Memorial Hospital. Dr. Funk was the medical director of North Fork Ambulance Association for 10 years prior to opening his practice.
Robert Van Gemert, M.D.
Robert Van Gemert, M.D., is the medical director at the Senior CommUnity PACE Health Center in Montrose. He earned his Doctor of Medicine from the University of New Mexico. He also holds a bachelor’s degree in chemistry from the University of New Mexico. Dr. Van Gemert has had a private practice in Montrose since 1978. He is the medical director at Valley Manor, a skilled nursing home and rehab center. He is also the medical director at Volunteers of America Home Health. Dr. Van Gemert was Chief of Staff at Montrose Memorial Hospital in 1986 and 1991. Dr. Van Gemert was a preceptor at the A.H. Gould Family Practice Clinic. He continues to be an in-office preceptor for residents and students.
Cheryl Dean
Eckert Health Center Director
Cheryl Dean is the center director for the Senior CommUnity Care PACE Health Center in Eckert. She brings 17 years of health care experience 12 years of long-term care experience to the team. Dean is a registered dietitian and worked in that field for 15 years, but felt drawn to working with the elderly. In an effort to influence the care that the elderly receive, Dean obtained her Nursing Home Administrator’s License in Colorado and began a new career as a nursing home administrator in 2007. After three years as an administrator she joined the Senior CommUnity Care team. She is enthusiastic about expanding the program to serve more seniors on the Western Slope. Dean earned her bachelor’s degree from Cornell University and completed a Harvard-Affiliated internship at Brigham and Women’s Hospital, in Boston, Mass.
Christie O'Hanlon
Montrose Health Center Director
Christie O'Hanlon is the center director for the Senior CommUnity Care PACE Health Center in Montrose. She brings more than 20 years of experience in various areas of geriatric and long-term care to the care team. She started her career working as a Certified Nursing Assistant on Alzheimer’s and mental health care units while attending college. She graduated from the University of Northern Colorado with a gerontology degree. O’Hanlon has served as activity director, social worker, retirement community administrator and adult day program manager. O’Hanlon earned her Master of Business Administration in health care management from Regis University in Colorado. Prior to joining Senior CommUnity Care, O’Hanlon worked for McKee Medical Center/Banner Health as director of geriatric and volunteer services for the community hospital.
Monica Rawson, FNP-BC
Nurse Practitioner
Monica Rawson joined the Senior CommUnity Care staff in February 2009 as the nurse practioner. She is a board certified family nurse practitioner with more than 14 years of experience. Rawson graduated from Mesa State College with a Bachelor of Science in nursing. She earned her Master of Science in nursing from the University of Colorado, Colorado Springs. Prior to coming to Senior CommUnity Care Rawson worked in acute care settings on the Western Slope. She is committed to caring for the geriatric population and has lived on the Western Slope for most of her life.
Leslie Quon
Human Resources Manager
Leslie Quon is the human resources manager for Senior CommUnity Care. She brings more than 15 years of experience to the team. She has worked in a variety of industries, including health care, insurance, banking, manufacturing and retail. Quon attended Sawyer Business College in Pasadena, Calif.
Carol Keller, RN
Director of Quality Assurance
Carol Keller oversees the quality assurance program for Senior CommUnity Care, assuming responsibility for compliance with state and federal regulations. Keller has experience in all aspects of nursing from long-term care as a director of nursing, to hospital, hospice and home health care environments. Along with 15 years of nursing experience, she has experience in quality project management and technical writing. Keller holds an associate's degree in nursing and a bachelor's degree in molecular biology. She is also certified in Life Care Planning, projecting life time services for persons with catastrophic injury. Keller has lived in Delta County for 15 years, providing quality health care to those residents.
Eva Veitch
Director of Outreach
Eva Veitch joined Senior CommUnity Care in March 2008 as the director of outreach. She has worked in health care 18 years, specializing in care for the elderly. She has extensive experience in long term care, assisted living and senior transportation. Veitch is committed to providing elders the best possible quality of life. She feels it is important to empowering our seniors to be as independent as possible. The PACE model speaks to everything health care should be for Veitch. Her health care experience in rural Western Colorado, and ability to develop relationships with elders, their care providers, local physicians and community resources supports the outreach efforts of the PACE program. Veitch has lived on the Western Slope more than 32 years and attended Mesa University.
Michelle Haynes
Accountant
Michelle Haynes joined Senior CommUnity Care as an accountant in 2008. She has worked in nonprofit health care settings for more than 20 years. She earned her bachelor’s in accounting and Master of Business Administration from the University of Louisiana at Monroe. After moving to Colorado, Haynes worked in the finance department of a community mental health center in the Denver metro area. Haynes and her family moved to rural southwest Colorado in 1997 where she served as executive director for a federally qualified community health center. She enjoys working with organizations that provide services to vulnerable, underserved populations, and views PACE as the model of quality and compassionate health care for the elderly.
