Senior Community Care is looking for individuals who want to become a valued member of a creative and caring team. Senior Community Care is pioneering a new program in Montrose and Delta Counties to help seniors remain in their homes while receiving expert care from a qualified team of professionals. This program is the first of its kind on the Western Slope and you can be a part of it.
Interested? We are looking for enthusiastic, qualified candidates with a minimum of one year’s experience working with the frail or elderly populations in the following positions:
Registered Nurses: Feel good knowing that you can provide skilled nursing care to participants in their homes, where they feel most comfortable, and/or in the clinic. RNs also review the plan of care and evaluate participant compliance, coordinating care with the interdisciplinary team.
Occupational Therapist: Our OT will provide clinical occupational therapy evaluation and treatment on the acute, restorative, or maintenance level, as well as integrate the occupational therapy treatment plan into the team Plan of Care. ACLS and home care experience preferred.
Recreation Coordinator: Seniors like to have fun too. The Recreation Coordinator will plan, organize, direct and participate in a comprehensive therapeutic recreation program tailored to the needs and limitations of program participants. Requires minimum of BA in Recreational Therapy or related field.
Certified Nursing Assistants: Provide and assist participants with activities of daily living, restorative and support care. May transport participants in Center Vans.
