How PACE Works
Senior CommUnity Care provides all health care services when a person enrolls with us.
Services are provided in our PACE Health Center, at home, or in another setting necessary to manage a person’s needs. We work closely with the family, the participant, and other caregivers to make sure all needed services are provided.
